Sliding Fee Scale
Gerald Family Care will provide essential services regardless of the patient’s inability to pay. We offer discounts based on family size and annual income. Patient eligibility for the program is decided by these factors. Sliding fee rates are based on Federal Poverty Guidelines. This policy and Sliding Fee Schedule program apply to all Gerald Family Care patients. No one will be denied access to services due to their inability to pay. The Sliding Fee Scale Policy and Fee Schedule are included here with the application and are also available at each location. To apply, print the application and submit the requested documentation.
